What you need to complete the form
Have these two documents in front of you before you open the form. If either is missing, you will have to start again.
eMedical reference letter
Obtained from the New Zealand immigration portal.
Valid original passport
A valid passport of a nationality eligible to apply for the New Zealand visa.
What to bring with you
What you bring that day decides whether the examination can be completed in a single visit.
Original documents
All the ones you used to complete the online form. Originals, not copies.
Glasses or contact lenses
If you wear them, bring them with you.
Applicants under 18
They come with their parents or legal guardians and with those adults' passports. Legal guardians also bring the documents proving guardianship.
No need to fast
None of the tests requires you to fast.

These four categories cover most New Zealand visas, both permanent and temporary. They are ordered by the applicant's age. Open yours.

- Medical examination.
- An IGRA tuberculosis test may be required (€175 extra).
- COVID-19 vaccination certificate, voluntary. If the child is vaccinated and you have kept the record, bring every dose.
- DNA test, only if the New Zealand Department of Foreign Affairs asks for it.

- Medical examination.
- COVID-19 vaccination certificate, voluntary, with the record of each dose if you have kept it.
- DNA test, only if the New Zealand Department of Foreign Affairs asks for it.

CAges 11 to 14€215
- Medical examination with a physical check: the applicant undresses to their underwear and wears a gown.
- You may ask for a witness to be present during the physical examination. It does not include a breast or pelvic examination.
- Chest X-ray, the same day and after the examination, at the Doctores Sales clinic (calle de Núñez de Balboa 88, 28006 Madrid), no appointment needed.
- The X-ray is taken during pregnancy as well. Tell the doctor at the visit if you are pregnant or think you may be.
- COVID-19 vaccination certificate, voluntary.
- DNA test, only if the New Zealand Immigration Authority asks for it.
- Urine sample.
Price: €215.
D15 and over€300
- Medical examination with a physical check: the applicant undresses to their underwear and wears a gown.
- You may ask for a witness to be present during the physical examination. It does not include a breast or pelvic examination.
- Chest X-ray, the same day and after the examination, at the Doctores Sales clinic (calle de Núñez de Balboa 88, 28006 Madrid), no appointment needed.
- The X-ray is taken during pregnancy as well. Tell the doctor at the visit if you are pregnant or think you may be.
- Blood test: eGFR and serum creatinine.
- COVID-19 vaccination certificate, voluntary.
- DNA test, only if the New Zealand Immigration Authority asks for it.
- HIV serology, only if you are applying for a permanent visa (€20 extra).
- Doctors, dentists and nursing staff working in healthcare: HIV and hepatitis B and C serology (€60 extra).
- Urine sample.
Price: €300, not counting the extra tests listed.
Prices of individual tests
The New Zealand Immigration Authority may ask for tests other than those covered by the four categories. These are the prices per test.

When the HIV test is required
These are the cases in which the test forms part of the examination.

How long your examination is valid
The medical examination is valid for 6 months
The six months count from the day it is performed. After that it expires and has to be repeated.
Public health risk assessment
Holders of a permanent visa granted outside New Zealand may be required to enter New Zealand before a first entry date (FED), which depends on the expiry date of their health clearance. If they have not entered by that date and the clearance has expired, they may have to repeat the medical examination.
